Issue:
Veteran A received an appointment letter intended for Veteran B that contained Veteran B's full name, partial SSN and home address. Veteran A did not realize he had Veteran B's letter until he returned for the follow-up appointment today and did not have an appointment. He went to his car and produced the letter. Update: 09/23/11:Veteran B will receive a letter of notification.
