This document contained the name, address, full SSN and the Protected Health Information (PHI) of one patient. Update: 08/08/12:The patient will receive a letter offering credit protection services.08/10/12:The patient died earlier this week. The Privacy Officer will prepare a next of kin letter for the Director's signature.
