Issue:

Veteran A went to the pharmacy window to pick up medication. When he got home, he realized that there was information sheet in the bag on Veteran B. The Privacy Officer (PO) is currently unable to determined what is on the information at this time, and will update the ticket as soon as possible. Update: 10/09/12:One Veteran will be sent a letter of notification.11/08/12:An appeal was filed. The DBCT reviewed and granted the appeal based on the evidence submitted that states that when the Veteran returned the paperwork, it contained Veteran B's full SSN. Veteran B will receive a letter offering credit protection services.

Outcome:

Education was provided to Pharmacy Technician to ensure that all information given to patient has only that patient's information in the bag.
