Issue:

Visitor on the grounds found a Veterans C & P File near a dumpster, lying against a pole behind our facility. The visitor gave it to his sister and she return it to the facility on 12/20/2012. Update: 12/21/12:Due to medical information and full SSN being exposed, Veteran A will be sent a letter offering credit protection services.

Outcome:

Credit Monitoring sent out to Veteran. Continue to follow up with site chief about UPS delivery. PO forward ticket and action to the senders site Privacy officer. After entering the PSET ticket- PO contacted the Clinical Support Chief and turn over the documents to her. I follow up with the civilian that brought the documents to one of our staff here. I ask Clinical Support Chief to find out if Fresno sign for the shipment or not. During this time frame the ticket came back with recommendation for credit monitoring. About 2 weeks later, we were able to get the tracking number and UPS tracking for the package. (See attachment) It indicated that it was not delivered to Fresno. I just got back from Leave yesterday and mailed out the credit monitoring and close out the PSET ticket. I plan to contact our Clinical Support Chief to follow up with the UPS situation.
