Issue:

Veteran A was given Veteran B's medication list at check-in. He was taken back to the triage nurse and noticed that the medications on the list were not his. He then looked at the name on the list and realized it was not his list. He returned it immediately to the triage nurse and it was promptly destroyed. He had it in his possession for less than 3 minutes. Update: 05/22/12:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.NOTE: There were a total of 20 Mis-Handling incidents this reporting period. Because of repetition, the other 19 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

The process for medication reconciliation at check in was reviewed with the new staff member. Education was provided as requested by supervisor.
