Issue:
A Release of Information (ROI) clerk was checking on the status of a UPS package sent that contained a copy of a Veteran A's medical records. UPS reported that the package is lost as per the tracking number and not delivered to the recipient nor was it returned to the sender. A claim was filled with UPS. The package contained Veteran A's name, full SSN, date of birth and protected health information (PHI). Update: 03/31/13:This is not the fault of VA, but due to the loss of the Veteran's SSN, Veteran A will receive a letter offering credit protection services.
