Issue:
Based on interview and record review, the hospital failed to safeguard one patient's (Patient A) protected health information (PHI). This resulted in the unauthorized access of Patient A's health information. Findings:The facility Breach Report, dated 6/19/13, was reviewed on 6/20/13. The report indicated Scheduling Clerk 1 contacted the next of kin for Patient A in error. The clerk was attempting to contact Patient A to have her complete the pre-procedural registration process. The facility investigated the incident and determined that the clerk used information from 2007. The clerk received counseling and training.
