Issue:

Veteran A called the VA Maury County Community Based Outpatient Clinic (CBOC) to report that when he received a Patient Notification Letter of Test Results in the mail, he also received the same type letter belonging to Veteran B. Veteran A informed the CBOC that he placed the letter of the Veteran B in an envelope and mailed it to him. The Privacy Officer (PO) contacted Veteran A and he confirmed he did place the letter belonging to Veteran B in a separate envelope and mailed to his home address. Information of Veteran B included his full name, home address and the results of a laboratory test.

Outcome:

11/24/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
