Issue:

On 08/01/12, a VA Long Beach Healthcare System (VALBHS) Licensed Vocational Nurse (LVN) from Primary Care/Specialty Care placed a roster containing 20 Veterans full names, social security numbers and phone numbers in an unsecured desk drawer prior to leaving for her lunch break. The LVN returned from lunch and the documents were not in the desk drawer. The LVN followed up with co-workers and her supervisor to see if anyone had removed the roster. It was determined that no employees removed the roster. Update: 08/03/12:All 20 Veterans will be sent letters offering credit protection services.

Outcome:

Person responsible for not properly safeguarding PIIand PHI has been re-educated on the importance of safe guarding PII/PHI.
