Issue:
A notebook with Microbiology result slips was found in old Emergency Room (ER) which was decommissioned upon new Emergency Department standing up. Space is limited access to VA Facilities management staff, VA Construction Contracting Staff, and VA Police. VA Police are in process of moving into some space in area and found in one room a spiral notebook with lab result slips had apparently been left behind by ER staff when they moved to new space. Number of patients involved: 41. The access to the space is limited, no public access. Update: 12/08/11:Letters offering credit protection services will be sent to 41 Patients.
