Issue:
The Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) was notified on 06/05/15 that the BVAMC Pharmacy Service discovered medication was shipped to an incorrect address by one of their controlled substance Pharmacy technicians. Veteran B was delivered medication belonging to Veteran A on 05/27/15. Veteran B returned the medication to the BVAMC Pharmacy Service.
Outcome:
06/05/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
