Issue:
Two inpatients went on pass on 09/23/11. The pharmacy delivered both sets of pass medication to the floor on 09/23/11. Veteran A was sent home with his medications but one medication of Veteran B's was sent along with him. The caregiver of the Veteran A contacted the inpatient pharmacist on 09/23/11, stating that they had Veteran B's medication. This was reported to Privacy Officer on 10/05/11. Update: 10/06/11:Veteran B will be sent a notification letter.
Outcome:
Pharmacy staff was re-trained on the importance of ensuring accuracy when dispensing medication.
