Issue:

Veteran A picked up his prescription at the pharmacy window on March 25, 2011. He was given the right bottle of medication, however, his bag contained the paper work information for Veteran B. He was instructed by Pharmacy to destroy the paperwork. Update: 03/25/11:The Chief of Pharmacy wrote that the information would have included Veteran B's name and address. It would also have had the drug information for the drug he received. It also may have had a profile listing of the other medications this patient was on. There was no social security or last 4 in the paperwork Veteran A received.Veteran B will receive a letter offering credit protection services.

Outcome:

New Process set up in which a double check is used before medication or information is given to a patient at the pharmacy window.
