Issue:

On May 17, 2013 the Pharmacy Supervisor reported to the Privacy Officer (PO) that Veteran B turned in three medications in two bottles. They were received by mail on May 16, and were intended for Veteran A. The medications were mailed by the Pharmacy to Veteran A on 5/14/2013. Pharmacy reviewed the address change log where Veteran As address was changed to that of Veteran B on 5/8/2013 in the clinic. Review of both Veterans address in UPS database by Pharmacy showed small differences from VHA data. Messages were left to precisely identify addresses for both Veterans. Veteran As medications will be mailed again upon address verification. None of the medications were narcotics nor highly priced. Recovered documents will be provided to PO. Additional notification and investigation to ensue

Outcome:

05/20/13: Veteran A will be sent a letter of notification. 06/05/13: This was determined to be non-HITECH reportable by VHA Privacy Office.
