Today, October 10, 2012 Veteran A was given a urine specimen cup which belonged to Veteran B. The label, on the specimen cup, contained Veteran Bs full name, full SSN, and DOB. The specimen cup was returned to staff for processing. Update: 10/11/12:Veteran B will be sent a letter offering credit protection services.
Outcome:
10/17/12 - Employees counseled on the importance of safeguarding Veteran's PII. CM letter mailed.
