Issue:

On November 17, 2014, A Veteran/Employee (Supervisor in the Business Office), when picking up his medications from the Outpatient Pharmacy was provided a printout titled "Confidential Meds Reconciliation OPT Active Summary." At that time he did not notice the paperwork belonged to another Veteran (Veteran B). On November 18, 2014, upon checking the document, he realized he was provided a print-out of another Veteran. Upon noticing the error, he immediately notified the Privacy Officer and provided the print-out. The print-out contained the full name, full SSN, DOB and all medications of the other Veteran.

Outcome:

11/18/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
