Issue:
It was reported to the Privacy Officer that a patient returned a list of patient appointments for the Bemidji Community Based Outpatient Clinic (CBOC). The patient stated that when he saw his provider on 05/22/12 he was given educational material at the end of his appointment and the last page was a list of appointments for that day. The list contained the names, phone numbers and full SSN of 5 other Veterans. No other personally identifiable information (PII) was disclosed. Update: 06/13/12:Five Veterans will be sent letters offering credit protection services.

The provider was talked to about safe guarding PHI/PII.
