Issue:

On 10/29/2012 the Records Center and Vault (RC&V) received a package from Biloxi VAMC. The package was shipped via United States Postal Service (USPS) and contained two files on one Veteran. The packaging was damaged during shipment. USPS placed the two files in a clear plastic bag that was not sealed, and placed a USPS sticker on the bag which states the package was damaged during shipment. The Veterans full name and full SSN are clearly visible through the bag. It is unknown at this time how many files were mailed. An investigation is being conducted. Update: 10/31/12:Even though this is USPS fault, the Veteran's full SSN was potentially compromised, so he will be sent an offer for credit protection services.

Outcome:

VA Gulf Coast modified the medical records mail out process from the United States Postal Service certified mail to UPS.
