Issue:

On 05/21/13, Veteran A was seen at the JACC Community Based Outpatient Clinic (CBOC) and received information from the provider for educational purposes. The information the provider handled to Veteran A included 12 pages of medical information belonging to Veteran B. Veteran A left the clinic and discovered the information he received contained copies of Veteran B's health information. On the same day, Veteran A called the clinic and spoke with the provider's nurse to report the incident. The nurse told Veteran A not to worry about receiving another Veteran's medical information and to return the documents upon their next clinic visit. Veteran A reported to the clinic on 06/21/13 for an appointment and notified the clinic clerk of the incident. Veteran A relinquished the documents to the clerk. The incident was reported to the Privacy Officer by the clerk's supervisor. The Privacy Officer reviewed the documents and notes the medical information belonging to Veteran B had been faxed to the provider earlier in the day on 05/21/13. The progress notes belonging to Veteran B documents an emergency room visit at a local military base. At this time, it is believed the nurse who advised Veteran A to return the documents at their next visit is no longer employed at the CBOC.

Outcome:

06/21/13: Veteran B will be sent a letter offering credit protection services.
