Mentioned in a privacy incident report created by the U.S. Department of Veterans Affairs on January 5, 2015. Also cited in 102 other reports.

Report ID: PSETS0000113436, U.S. Department of Veterans Affairs

Reported Entity: PHOENIX AZ - 644

Issue:

Today, 01/05/14, the Privacy Officer (PO) received multiple reports of prescriptions which were mailed to the wrong Veterans address. The medication was mailed in April 2014 to Veteran A which Veteran B reported was received at his address. Veteran B reports that he took the medication until recently, which he reported to Pharmacy last Friday. Upon Pharmacy call, Veteran B will not relinquish remainder of medication or prescription bottle; education materials were disposed upon receipt. Root cause: On-site address change for Veteran A to match Veteran B which caused Consolidated Mail Outpatient (CMOP) Pharmacy to dispense the prescription to the incorrect address. This is an atypical delay in reporting and retention of other Veterans protected health information (PHI). The PHI exposed was the Veteran's full name and medication. Additional investigation and notification, to ensue.

Outcome:

01/05/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
