Issue:

A VA employee reported that her car was broken into over the weekend and her work bag was stolen. Her work bag contained the following: 1. Scheduling calendar 2. Clipboard with phone log, VA phone numbers, notes 3. Patient handouts 4. Paystubs and timesheets 5. Folder with HPC resources 6. Folder with educational materials 7. Some passwords but no usernames (except TMS Username and Password) 8. Headset The employee reports that interspersed throughout her paperwork were patient last names and last 4 digits of the SSN. The employee reports that she does not recall having any more detailed information on patients in her bag.

Outcome:

06/01/15: Employee reported that her scheduling book contained the last name, last 4, date of appointment and primary diagnosis and that the phone log contained last names, phone numbers and reason for the calls. I have asked the provider to provide me a list of her schedule appointments for the clinics she runs as this will hopefully provide the names of the veterans who were in her scheduling book. 06/18/15: A total of 24 Veterans will be sent HIPPA nitification letters.
