Employee A reported finding documents containing 10 Veterans Names, last four of SSN and diagnosis (7332 diagnosis) in an open conference room unattended. The documents were left unattended for approximately 30 days in an unlocked room open to employees and other staff. The employee who found the documents turned them over to the Privacy Officer.

Outcome:

09/06/13: The conference room was unlocked and un-monitored. The general public would have had access to it. All 10 Veterans will be sent letters of notification.
