Issue:

The Community Based Outpatient Clinic (CBOC) at this facility sends prescriptions in a locked blue bag to this facility. They put a check-off list in the bag of what information is included in the locked bag. The person that receives this information will deliver documents to the providers, mailboxes, scanning to Release of information (ROI), and the prescriptions to Pharmacy. On 01/04/12, a bag was delivered to this facility and the person receiving delivered the items to the appropriate places, but does not remember who exactly she handed the information to in the Pharmacy at the window (there was no signoff for this portion). A Report of Contact was written by the person who received the information. Pharmacy also did a Report of Contact. Once the person who processes the prescriptions receives them, they enter that information on a hare point site to track them.. It was not updated so they did not receive this information. The prescriptions were recreated and processed so the patient were taken care of. Update: 01/11/12:Due to the full SSN and medical information being exposed, Veteran A will be sent a letter offering credit protection services.02/09/12:Revised count of 6 Veterans.

Outcome:

Education was given to employees. A new process was put into place to help eliminate any missing documents in the future.
