Today, 04/10/15, the Privacy Officer (PO) received a report of the disclosure of paper protected health information (PHI) in the form of one page of Veteran A's information to Veteran B. Veteran B presented this document to the Lab. The document was recovered. The document contained Laboratory studies scheduled with upcoming appointment dates and locations. The data fields contained sensitive 38 USC 7332 diagnoses, Veteran A's full name and partial SSN. Additional notification and investigation, pending.

Outcome:

04/13/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
