Issue:

Lake City Pharmacy received a call this afternoon from Veteran A inquiring about the date his pain medication was mailed out or when it was going to mailed out. Upon researching, Pharmacy discovered that Veteran A's prescription had been mailed out to Veteran B. Pharmacy contacted Veteran B and he confirmed receiving a package of medication that he did not request and was not his. Veteran B was asked to package up the medication and all paperwork that had been sent to him by error and return it to the Pharmacy via UPS return services. Veteran B has agreed to do this and will have the package ready for pick-up by UPS on Monday.

Outcome:

03/09/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
