Issue:
Veteran/Employee A informed the Privacy Officer that he had requested his medical records from Release of Information (ROI) and received another Veteran's large packet containing the other Veterans medical record. He stated he returned the packet to ROI, however does not remember any information about the other record. He stated he never received his medical information. Update: 12/14/12:Veteran stated this incident occurred back the end of July, 2012. The DSS Supervisor was contacted to conduct a search. She stated no ROI employees reported or recalled any records being returned the end of July or August. The ROI DSS Manager program system was reviewed and reflects that Veteran's request for medical information was processed 7/24/12 and reflected it was mailed to the correct address. The information mailed was the entire medical record (full name, date of birth, SSN, medical information). Staff are unable to determined whether the Veteran's record was lost in the mail or mailed incorrectly. Staff is unable to determine the identity of the mismailed Veteran information that was returned .12/14/12:Veteran/Employee A will be sent a letter offering credit protection services.
Outcome:
New process started where the ROI supervisor or acting will be notified of any records being returned.
