A Primary Care Nurse Practitioner provided the lab results of Veteran B to Veteran A during his clinic visit. Veteran A telephoned and was instructed by clinic staff to destroy the wrong lab document. Veteran A states he has shredded the document. Update: 04/17/13:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.06/05/13:This was determined to be HITECH reportable by VHA Privacy Office.

Outcome:

Education was provided. Notification letter has been mailed.
