Issue:
A Clerk at a Community Based Outpatient Clinic (CBOC) received a call from Veteran A, stating that he received a specimen container in the mail via UPS. Veteran A collected the specimen and was preparing to send it back when he noticed that there was a label on the container with Veteran B's full name and full Social Security Number. Lab staff were notified of the incident. The specimen was not processed once it was returned to the Medical Center. Update: 11/28/12:Veteran B will receive a letter offering credit protection services.
Outcome:
Process for mailing lab specimen cups is currently being evaluated and revised to prevent similar incidents from occurring in the future.
