Based on interview and record review, the hospital failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the hospital's Patient Relations (PR) was conducted on 2/28/13 at 3:55 P.M. The PR stated that Patient 1 and Patient 2 were both seen at the hospital for a diagnostic test but on different days. The PR stated that the staff who mailed the test result report inadvertently placed Patient 1's test result report with Patient 2's report and mailed both results to Patient 2. The report that was inadvertently mailed to Patient 2 indicated Patient 1's name, home address, diagnostic test, and the test result.
