Issue:

Two patients are on the same medication The first dose has to be administered by our staff. The nurse accidentally gave Patient A the medication belonging to Patient B and vice versa. The remaining medicine was then sent home with the patient. This medicine had the other patient's name, address, medication name and doseage with the packaging. Update: 11/02/11:Patient A and Patient B will both be sent a notification letters due to their names and PHI being disclosed to each other.

Outcome:

Staff reminded to be more cautious when administering medication.
