A VA employee placed a medication label (full name, full SSN, and date of birth) on a package that was sent out in the U.S. Mail. Update: 09/04/12:The patient will receive a letter offering credit protection services.
Outcome:
Staff was made aware of the need to place envelopes for Pharmacy in different bins than mail to be sent to the UPS.
