Issue:

A Release of Information (ROI) employee released Veteran A's discharge summary from October 7, 2009 on April 5, 2010. The ROI employee could not recall the details of the release. A Court Order for Appointment of Guardian ad Litem was scanned into the Veteran's medical record on April 12, 2010. However, this court order is for the Veteran A's son, not the Veteran. The October 7, 2009 discharge summary was released to the Guardian ad Litem but this is not documented in ROI package. The ROI section cannot locate the original request or produce any documentation that the Veteran had requested the October 7, 2009 discharge summary. Update: 08/30/11:One Veteran will be sent a letter offering credit protection services, due to full name and full SSN being exposed.

Outcome:

VA employee was issued a written counseling statement on April 1, 2011 by his supervisor. Additionally, actions to identify Veterans who request information from ROI in person is being better documented.
