Issue:
Veteran A was issued a new CPAP (breathing machine issued by Sleep Lab) as Veteran A was having problems with the machine working properly. During the course of the conversation the nurse taking care of Veteran A examined the machine and realized that the card (stores information on breathing, air leaks, how long the patient wears the machine etc.) issued to Veteran A belonged to Veteran B. The card also contained the full name and last 4 of the SSN of Veteran B. Veteran A is an Ann Arbor VAMC patient and Veteran B, whose card was given to Veteran A is a Detroit VAMC Veteran. Update: 12/20/12:Veteran B will be sent a letter of notification.
Outcome:
Staff have been trained to verify CPAP (continuous positive airway pressure) download cards before issuance.
