Issue:

The two page Quality Management (QM) Protected Peer Review was shared with 14 providers for staff education only. The document contained one patient's protected health information (PHI) and identified three recommended actions 1. most practitioners would have managed case differently; 2. process for reporting abnormal finding to be reviewed and modified; and 3. recommended additional peer review for a named provider. The Peer Review discussed a total of four providers review of care. The Peer Review documentation was clearly identified as QM protected, retrieved from providers after discussion, and shredded. The use was made in good faith and within the scope of authority and did not result in further use or disclosure in a manner not permitted by VA policy. The Sensitive Personal Information (SPI) was disclosed by persons who are authorized to access SPI at VA.

Outcome:

05/27/15: The Incident Resolution Service Team has determined that there was a policy violation. No data breach has occurred.
