Patient A received a reminder letter for his own appointment and a second letter intended for a Patient B. The Privacy Officer (PO) is unsure if these letters still have the last four digits of the SSN on them. The PO will ask Patient A to return the letter to determine the details. Update: 01/02/13:The Privacy Officer has been unsuccessful in contacting Patient A to verify the information contained in the letter. Patient B will be sent a letter of notification.
