Issue:
Patient A received his prescription package which was not sealed properly and was missing one of his medications. Patient A's name and type of medication was compromised. Patient A reported the incident to the Medical Center and a replacement has been requested for him. Great Lakes Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that this was a CMOP automated packing machine error. The sealer setting was adjusted on the automated packing machine to ensure it is sealing all packages correctly. Update: 01/18/13:Patient A will receive a HIPAA letter of notification.
