Issue:
A Veteran called and reported that when he was recently discharged from the VA Nashville Campus, he was provided his discharge instructions along with other documents. After he was home and reviewing these documents, he then noticed he received a document belonging to another Veteran. After discussion with this Veteran, the Privacy Officer believes the document belonging to the other Veteran was an Appointment List. The Privacy Officer did arrange for the return of the document of the other Veteran. Upon receipt, PO will update with what PII was included on that document. Update: 01/16/13:Due to full SSN and Medical information being exposed, Veteran B will be sent a letter offering credit protection services.2/1-13:The Facility received the documents back they discovered that the veteran whose information was disclosed has expired. next of kin notification letter will need to be sent.
Outcome:
NOK Letter mailed on 2/5/13 and redacted copy uploaded. It was determined it was a human error that created this incident. This issue was referred to Nursing Service to follow-up with the employee involved. Nursing Service will also provide education/training in their next staff meeting.
