Based on staff interview, clinical and administrative document review the facility failed to keep Protective Health Information (PHI) confidential when Patient 1's prescription was given to Patient 2.This failure placed Patient 1's PHI at risk for unauthorized use.On 9/5/14 at 08:49 a.m., Staff 1 stated on 6/21/14 at 12 a.m., the prescription for patient 1 was given to patient 2 by mistake.On 9/5/14 at 08:49 a.m., staff 1 stated patient 1's PHI breached clinical record included name, telephone number, emergency contact, and prescription.The hospital policy and procedure titled "Use and Disclosure of Protected Health Information" undated, indicated "Adventist Health entities will review , revise, or develop facility-specific policies and procedures addressing the Rule requirements related to the appropriate use and disclosure of protected health information (PHI).
