Issue:

Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) received information from BVAMC Fiscal Service employee that Veteran A came to her office window stating he received another Veteran's (Veteran B) travel voucher. The BVAMC Fiscal Service employee kept the voucher belonging to Veteran B and sent Veteran A to the Travel Clerk to obtain the appropriate voucher. The voucher contains the full name, address, and SSN of Veteran B

Outcome:

05/29/15: The Incident Resolution Service Team has determined that there was a privacy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise, as the Veteran returned the document to VA as soon as he noticed it wasn't his. As he was signing the document at the window, he noticed it was not his and immidiately returned it to the clerk at the window.
