Issue:

Mt. Pleasant, SC Police Department contacted our Pharmacy regarding prescription bottles being found in a dumpster. The bag was still tied and in the same condition as when it left the facility. The bag contained approximately 250 empty pill bottles with names removed but prescriptions listed. This bag was supposed to go to the recycle center. Also two Veterans names and address and two names and prescriptions were inside the sealed bag. Update: 09/24/12:Two (2) Veterans will be sent a HIPAA notification letter due to name and PHI being exposed. Two (2) Veterans will be sent a General notification letter due to name and address being exposed.

Outcome:

Pharmacy Supervisor will discuss this incident at next staff meeting. Supervisor will stress how a careless mistake can have wide spread ramifications. Supervisor will also talk one-on-one with employee who made the mistake.
