Issue:

An outside Attorney's Office telephoned the Medical Records section reporting they did not receive a Veteran's information. The information was mailed via UPS on 11/15/12 to the Attorney's Office and has a tracking number and was signed for there. The package weighed between 20 and 30 pounds. The Privacy Officer (PO) is investigating. Update: 03/07/13:The PO is awaiting an interoffice mailing with the documentation and signature. Their plan is to attempt to read the signature and contact the Attorney's office and ask them to conduct a search for the package. 03/18/13:The proof of delivery does not required signature. The PO placed a tracer on this message once she saw the proof of delivery form. We are waiting for the investigation report from UPS. They expect to have this information to us within 4-8 days.03/28/13:UPS could not locate the package nor could they locate any information about the information in the "left at" line on the Proof of Delivery. Investigation has concluded with package not recovered and no further information available. Even though this was not a VA error, because the records contained the Veteran's name, date of birth and full SSN, VA will send him a letter offering credit protection services.
