Issue:
A nurse inadvertently mixed mailing instructions from the primary care provider. Patient A's information was mailed to Patient B after their walk-in visits on 05/03/12. The information included Patient A's full SSN and medication list. Update: 05/07/12:Patient A will be sent a letter offering credit protection services.NOTE: There were a total of 28 Mis-Mailed incidents this reporting period. Because of repetition, the other 27 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The employee responsible and all staff have been re-educated.
