Issue:
A VA employee provided Veteran A with Veteran B's discharge instruction sheet. The discharge instruction sheet contained the full name and procedure of Veteran B. The VA employee recognized the error and contacted Veteran A who confirmed he had received Veteran B's discharge sheet. Veteran A stated he destroyed the discharge sheet in his wood burning stove. Update: 02/11/13:Veteran B will be sent a HIPAA notification letter.
Outcome:
The staff member received administrative action; re-educated about patient safety and our verification policy for Veterans including checking patients' armband, asking the patient to state his/her name in full, and social security number in full; repeated Privacy and HIPAA training and VA Privacy and Information Security and Rules of Behavior training. HIPAA notification letter sent to Veteran and redacted copy uploaded.
