Issue:
Veteran/Employee A reported on two separate occasions that her coworker received copies of her information. Approximately three weeks ago Veteran/Employee B received Veteran/Employee A's appointment letter in the mail. Veteran/Employee B notified her co-worker and gave her the document. On October 10, 2012 Veteran/Employee B was mistakenly given a copy of Veteran/Employee A's report from Employee Health. Veteran/Employee B discovered the error while reviewing the document. Veteran/Employee A reports that on both occasions her co-worker made her aware of what happened and returned the information to her. Veteran/Employee A notified the Privacy Officer of her concern about the security of her information because of the two events occurring so closely. Update: 10/16/12:Veteran/Employee A will be sent a HIPAA letter of notification.

Additional privacy training to be provided to the Employee Health staff regarding the review of records process.
