Issue:

On 12/12/11, Pharmacy Service received a telephone call from Veteran A who was reporting that he noticed he received medications for Veteran B in the mail. Once Pharmacy Service researched, it was determined that on 12/2/11, VA Tennessee Valley Health System Pharmacy personnel addressed the UPS label incorrectly with the wrong Veteran's name and address and mailed to Veteran A in error. The personal information on the package included Veteran B's full name, home address and name of the medication. Update: 12/16/11:Veteran B will receive a letter of notification.

Outcome:

It was determined it was an employee error occurred when preparing the medication for shipment. PO has certified the employee involved has been educated and re-trained on the importance of accuracy when prepping medications for shipment. Veteran A has returned the mis-mailed medication to Pharmacy Service and Veteran B has now received his medication.HIPAA Notification Letter to be mailed 12/23/11.
