Issue:

The Consolidated Mail Outpatient Pharmacy (CMOP) in Leavenworth received a report from the VAMC stating Patient A received, in addition to her own supply, a supply intended for Patient B. Both patients had orders processed on the same date and time by Medline. Patient A has been instructed by the VAMC in the proper disposal of items received in error and Medline has replaced item intended for Patient B. Medline reports the error occurred during the mail label application and packaging process. They have forwarded to the appropriate supervisor for employee follow up. Update: 09/23/11:Veteran B will be sent a notification letter.NOTE: There were a total of 2 Mis-Mailed CMOP incidents out of # total packages (# total prescriptions) mailed out for this reporting period. Because of repetition, the other 1 is not included in this report, but are included in the "Mis-Mailed CMOP Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter.

Outcome:

Medline reviewed the incident and conducted retraining of their staff.
