Issue:

A VA Social Worker attended a stand down homeless program for Veterans. The VA station car was in an accident and returned to our VA garage. Personally identifiable information was left in the car. There are non-VA employees - compensated work therapy staff that have access to our VA garage. All personal information was still in vehicle when it was discovered that the information was overlooked and not gathered up by the Social Worker. Information that was accessible was patient name and some full social security number and partial social number. A total of 5 Veteran's personal information was discovered in the VA vehicle. Update: 02/10/12:Five Veteras will be sent a letter offering credit protection services.

Outcome:

Employee was reminded to safeguard personal patient identification information especially when off VA government grounds.5 Veterans involved in case - 1 credit monitoring letter attached.
