Issue:
Based on hospital document review and interview, the hospital failed to prevent the disclosure of protected health information (PHI) to unauthorized entities for Patient Qq. In addition, the hospital failed to notify Patient Qq and the Department of the disclosure of the PHI within five business days of their knowledge of its occurrence. Findings:Review of hospital documentation showed on 12/10/11, Patient Qq's PHI was disclosed to unauthorized individual(s). The hospital's documentation showed the Patient Financial Services department received a call from Patient Qq on 12/10/11. The patient stated his hospital bill was given to him by his employer. The employer opened it before giving it to the patient. The PHI disclosed Patient Qq's name, address, bill charges and reason codes for the hospital visit. The Department and Patient Qq were notified of the breach of PHI on 12/23/11.During a telephone conference, on 4/6/12 at 0930 hours, the hospital's Manager of Regulatory Compliance verified the above breach occurrence and notification dates.
