Issue:

Patient A and Patient B were seen at the Chattanooga Community Based Outpatient Clinic (CBOC). On the day of the appointment, Patient A and Patient B left before seeing the clinic nurse. Patient A later received the medication reconciliation list, which included Patient B's full name, SSN, and date of birth, in the mail. Patient B has not received any information in the mail. The Privacy Officer confirmed that Patient A returned Patient B's information to the Chief, Medical Director at the Chattanooga CBOC. Update: 03/08/11:Patient B will receive a letter offering credit protection services.

Outcome:

3/18/11 - The mis-mailed document was due to a human error and the nurse in question has been counseled regarding the importance of protecting a Veterans Personal Identifiable Information (PII). Additionally, the Privacy Officer confirmed the mis-mailed document was returned to the Chattanooga CBOC. CM letter mailed.
