Issue:

The Privacy Officer (PO) received an email from the Health Resource Center (HRC) stating that a billing statement was mismailed to the City of San Luis Obispo employee who called and returned the information to the HRC. The email stated that it was probably mailed by the National Print Center based upon information input into our system by our medical center staff and that we should correct it by entering "address unknown." The information in the letter contained Veteran A's first and last name and medication information. Update: 04/11/11:Veteran A will be sent a notification letter.

Outcome:

The Veteran's address has been confirmed and updated. This was no fault of VA.
